The forms for the Commuter Card Request and the Student Discount Card Request are available online.
By downloading and filling out the forms from the comfort of your home, it will be possible to deliver them directly to the ticket office, in case of need, to obtain the desired card.
How to proceed:
- Download the form of your interest:
- Fill in all the required fields.
- To request the issue of the Commuter Card, it is necessary to attach to the form:
- A photocopy of a valid identity document.
- A recent passport photo.
NOTE:
For employees:
You must submit a declaration on letterhead provided by your employer, attesting to your commuting condition.
For the self-employed:
It is necessary to provide a declaration of commencement of activity duly registered with the Revenue Agency.
- Important for all commuter workers: All commuter card holders are obliged, by the end of each month, to send travel receipts to the pendolari@delcomar.it email address, otherwise the card will be collected.
